New Counseling Client Forms:

Once submitting a counseling inquiry through the website or by email, the clinical director will reach out to begin the intake process. You will be emailed the new client intake packet to complete and email back to thefletcherccc@gmail.com, prior to your scheduled initial session.

If you will be receiving health and wellness coaching, please print and sign these forms and return by email (thefletcherccc@gmail.com) prior to your scheduled initial session with the health and wellness coach.
