Please complete the New Client intake form prior to your first session and email back to the counseling center at: thefletcherccc@gmail.com. 

 

If you have a minor who is receiving counseling services, please also complete the Parent/Guardian consent form and return it as well to: thefletcherccc@gmail.com.
